HTTP 500 Internal Server Error with Auth_phpBB

I constantly get this error when I try to log into a newly created phpBB3 account from the Mediawiki login page.
This happens on my own local web server (php mod based) as well as on my Dreamhost account (php cgi based).
Is anyone else getting this error and what am I doing wrong?

This normally means PHP crashed or is miss configured. It can also be caused by a permission issue. Or a whole bunch of other stuff. If I had to guess I would say it's because the plug-in is trying to use an encryption/hash method that has not been compiled into your php install. But thats just a guess.

I have found a way to replicate the problem, but not the solution.
The problem occurs when a new phpBB3 user is created with the password set to the same user name.
When you log in from the wiki, you get this error.
When the password is set to something else, this error does not occur.
This only happens on a newly created user.
This error does not happen if you change the password later to match the user name.

Now, is there a way to limit the password from being the user name in phpBB3?

So if I make a new user "Test" with the password "Test" in phpBB and then go to MW and login w/ Test/Test you get the 500 error?

But had I done the same thing w/ Test/Pass. I would not get an error?

Then after logging into MW once I can change my pass to Test and now Test/Test will work w/o crashing?

Well, the exact procedure I used was:

-Go to MW login page and follow link to redirect to phpBB3 page to create new user "Test1" with password "Test1".
-Go back to MW login page and log into newly created user Test1/Test1.
-You get the 500 error.

If instead:

-Go to MW login page and follow link to redirect to phpBB3 page to create new user "Test2" with password "Pass".
-Go back to MW login page and log into newly created user Test2/Pass.
-You can log into MW.
-Go to phpBB3 login page and log into Test2/Pass.
-Change password to "Test2".
-Go back to MW login page and log into user Test2/Test2.
-You can log into MW.

I distinguish Test1 from Test2 so as not to reuse user names for this test.
